The advent of new genetic and genomic technologies may cause friction with the principle of respect for autonomy and demands a rethinking of traditional interpretations of the concept of informed consent. Technologies such as whole‐genome sequencing and micro‐array based analysis enable genome‐wide testing for many heterogeneous abnormalities and predispositions simultaneously. This may challenge the feasibility of providing adequate pre‐test information and achieving autonomous decision‐making. At a symposium held at the 11th World Congress of Bioethics in June 2012 (Rotterdam), organized by (...) the International Association of Bioethics, these challenges were presented for three different areas in which these so‐called ‘new genetics’ technologies are increasingly being applied: newborn screening, prenatal screening strategies and commercial personal genome testing. In this article, we build upon the existing ethical framework for a responsible set‐up of testing and screening offers and reinterpret some of its criteria in the light of the new genetics. As we will argue, the scope of a responsible testing or screening offer should align with the purpose(s) of testing and with the principle of respect for autonomy for all stakeholders involved, including (future) children. Informed consent is a prerequisite but requires a new approach. We present preliminary and general directions for an individualized or differentiated set‐up of the testing offer and for the informed consent process. With this article we wish to contribute to the formation of new ideas on how to tackle the issues of autonomy and informed consent for (public) healthcare and direct‐to‐consumer applications of the new genetics. BackgroundCRISPR-Cas9, a technology enabling modification of the human genome, is developing rapidly. There have been calls for public debate to discuss its ethics, societal implications, and governance. So far, however, little is known about public attitudes on CRISPR-Cas9. This study contributes to a better understanding of public perspectives by exploring the various holistic perspectives Dutch citizens have on CRISPR-Cas9.MethodsThis study used Q methodology to identify different perspectives of Dutch citizens (N = 30) on the use of CRISPR-Cas9. The Q-sort method (...) aims at segmenting audiences based on the structural characteristics of their perspectives. Participants individually ranked 32 statements about CRISPR-Cas9 and discussed their rankings in small groups. By-person factor analysis was performed using PQMethod. Participants’ contributions to the discussions were used to further make sense of the audience segments identified.ResultsFive perspectives on CRISPR-Cas9 were identified: (1) pragmatic optimism (2) concerned scepticism; (3) normative optimism; (4) enthusiastic support; and (5) benevolent generalism. Each perspective represents a unique position motivated by different ranking rationales. Sorting rationales included improving health, preventing negative impacts on society, and fear of a slippery slope. Overall, there is broad, but not universal support for medical uses of CRISPR-Cas9.ConclusionsResearch on CRISPR-Cas9 should prioritise the broadly supported applications of the technology. Research and public debates on CRISPR-Cas9, its uses, its broader implications, and the governance of CRISPR-Cas9 are recommended. A discourse that includes all perspectives can contribute to the embedding of future uses of CRISPR-Cas9 in society. Prolific research suggests identity associates with pro-environmental behaviours that are individual and/or group focused. Individual PEB is personally driven, self-reliant, and are conducted on one's own. Group focused PEB is other people-reliant and completed as part of a group. A wide range of identities have been related to PEBs. For example, a recent systematic qualitative review revealed 99 different types of identities studied in a PEB context. Most studies were correlational, few had an experimental design. However, the relationships between all (...) these identities and PEBs have so far not been tested quantitatively with meta-analytical techniques. As such, a clear overview of this field is currently lacking. Due to the diverse nature of the field, a priori hypotheses were not possible and relatively broad definitions of identity had to be used to encompass all types of identities and the diverse meanings of identity that have been included in PEB research. What prior theory did allow for was to assess the distinction between two main types of identity, namely how people label, describe, and recognise oneself individually, or as part of a group. Our overall goal was thus to assess the current state of knowledge on identities and PEBs. In 104 studies using a meta-regression following the preferred reporting items for systematic reviews and meta-analyses guidelines, our random-effects meta-analysis showed that the overall concept of identity associated with PEB with a medium Pearson's r. Furthermore, we found that individual identities associated more strongly with PEBs than group identities. The associations between individual and group identities were stronger when the identity and PEB were from the same category. Methodologically, the findings revealed that group identities and group PEBs were most strongly associated for self-reported rather than observed PEBs. Overall identity associated most strongly with group PEBs in the field rather than in the lab and in student- rather than non-student samples. We discuss the theoretical and practical implications. Scholars and policymakers have increasingly advocated to engage citizens more substantially in the development of science and technology. However, to a large extent it has remained unknown how citizens can contribute to technology development. In this study, we systematically characterized citizens’ contributions in the development of nanotechnology for health. We explored to which technology aspects citizens are able to provide suggestions on and on which values their suggestions are based. Fifty citizens in the Netherlands were asked to discuss different applications (...) of nanotechnology for health. They were divided over eight focus groups, which lasted approximately 6.5h each. The citizens were able to contribute to various technology aspects; they mainly focused on the technologies’ implementation and use, they paid some attention to the development and system aspects, and also addressed, but less, the design and communication aspects. Their suggestions were often derived from concerns about the potential effects of the technologies and were predominantly based on the values of well-being, autonomy, and privacy. (shrink)

Prenatal screening for foetal abnormalities such as Down's syndrome differs from other forms of population screening in that the usual aim of achieving health gains through treatment or prevention does not seem to apply. This type of screening leads to no other options but the choice between continuing or terminating the pregnancy and can only be morally justified if its aim is to provide meaningful options for reproductive choice to pregnant women and their partners. However, this aim should not be (...) understood as maximizing reproductive choice per se. Only if understood as allowing prospective parents to avoid suffering related to living with (a child with) serious disorders and handicaps can prenatal screening be a publicly or collectively funded programme. The alternative of moving prenatal testing outside the healthcare system into the private sector is problematic, as it makes these tests accessible only to those who can afford to pay for it. New developments in prenatal screening will have to be assessed in terms of whether and to what extent they either contribute to or undermine the stated aim of providing meaningful options for reproductive choice. In the light of this criterion, this article discusses the introduction of the new non‐invasive prenatal test (NIPT), the tendency to widen the scope of follow‐up testing, as well as the possible future scenarios of genome‐wide screening and ‘prenatal personalised medicine’. Literature on Shared Mental Models has been burgeoning in recent years and this has provided increasingly detailed insight and evidence into the importance of SMMs within specific contexts. However, because past research predominantly focused on SMM structure as measured by diverse, context-dependent measures, a consolidated multi-dimensional measure of perceived SMMs that can be used across diverse team contexts is currently lacking. Furthermore, different conceptualizations of the dimensionality of SMMs exist, which further impedes the comparison between studies. These key limitations might (...) hinder future development in the SMM literature. We argue that the field of SMMs has now matured enough that it is possible to take a deductive approach and evaluate the prior studies in order to refine the key SMMs dimensions, operationalizations, and measurement. Hence, we take a three-stage approach to consolidate existing literature scale-based measures of SMMs, using four samples. Ultimately, this leads to a 20-item five-dimensional scale – the Five Factor Perceived Shared Mental Model Scale. Our scale provides scholars with a tool which enables the measurement, and comparison, of SMMs across diverse team contexts. It offers practitioners the option to more straightforwardly assess perceived SMMs in their teams, allowing the identification of challenges in their teams and facilitating the design of appropriate interventions. The main alteration results from the way ITL extends Hart's linguistic turn towards ordinary language philosophy (OLP). Hart holds -- like Kelsen -- that law cannot be reduced to brute fact nor morality, but because of its attempt to reconstruct social practices his theory is more inclusive. By introducing the notion of law as an extra-linguistic institution ITL takes a next step in legal positivism and accounts (...) for the relationship between action and validity within the legal system. There are, however, some problems yet unresolved by ITL. One of them is its theory of meaning. An other is the way it accounts for change and development. Answers may be based on the pragmatic philosophy of Charles Sanders Peirce, who emphasises the intrinsic relation between the meaning of speech acts and the process of habit formation. (shrink)

Prenatal screening pathways, as nowadays offered in most Western countries consist of similar tests. First, a risk‐assessment test for major aneuploides is offered to pregnant women. In case of an increased risk, invasive diagnostic tests, entailing a miscarriage risk, are offered. For decades, only conventional karyotyping was used for final diagnosis. Moreover, several foetal ultrasound scans are offered to detect major congenital anomalies, but the same scans also provide relevant information for optimal support of the pregnancy and the delivery.Recent developments (...) in prenatal screening include the application of microarrays that allow for identifying a much broader range of abnomalities than karyotyping, and non‐invasive prenatal testing (NIPT) that enables reducing the number of invasive tests for aneuploidies considerably. In the future, broad NIPT may become possible and affordable.This article will briefly address the ethical issues raised by these technological developments. First, a safe NIPT may lead to routinisation and as such challenge the central issue of informed consent and the aim of prenatal screening: to offer opportunity for autonomous reproductive choice. Widening the scope of prenatal screening also raises the question to what extent ‘reproductive autonomy’ is meant to expand. Finally, if the same test is used for two different aims, namely detection of foetal anomalies and pregnancy‐related problems, non‐directive counselling can no longer be taken as a standard. Our broad outline of the ethical issues is meant as an introduction into the more detailed ethical discussions about prenatal screening in the other articles of this special issue. (shrink)

One in three university students experiences mental health problems during their study. A similar percentage leaves higher education without obtaining the degree for which they enrolled. Research suggests that both mental health problems and academic underperformance could be caused by students lacking control and purpose while they are adjusting to tertiary education. Currently, universities are not designed to cater to all the personal needs and mental health problems of large numbers of students at the start of their studies. Within the (...) literature aimed at preventing mental health problems among students (e.g., anxiety or depression), digital forms of therapy recently have been suggested as potentially scalable solutions to address these problems. Integrative psychological artificial intelligence (AI) in the form of a chatbot, for example, shows great potential as an evidence-based solution. At the same time, within the literature aimed at improving academic success, the online life-crafting intervention in which students write about values and passions, goals, and goal-attainment plans has shown to improve the academic performance and retention rates of students. Because the life-crafting intervention is delivered through the curriculum and doesn’t bear the stigma that is associated with therapy, it can reach larger populations of students. But life-crafting lacks the means for follow-up or the interactiveness that online AI-guided therapy can offer. In this paper, we propose to integrate the current literature on chatbot interventions aimed at the mental health of students with research about a life-crafting intervention that uses an inclusive curriculum-wide approach. When a chatbot asks students to prioritize both academic as well as social and health-related goals and provides personalized follow-up coaching, this can prevent -often interrelated- academic and mental health problems. Right on-time delivery, and personalized follow-up questions enhance the effects of both -originally separated- intervention types. Research on this new combination of interventions should use design principles that increase user-friendliness and monitor the technology acceptance of its participants. (shrink)

This paper inquires into the nature of intertheoretic relations between psychology and neuroscience. This relationship has been characterized by some as one in which psychological explanations eventually will fall away as otiose, overthrown completely by neurobiological ones. Against this view it will be argued that it squares poorly with scientific practices and empirical developments in the cognitive neurosciences. We analyse a case from research on visual perception, which suggests a much more subtle and complex interplay between psychology and neuroscience than (...) a complete take-over of the former by the latter. In the case of vision, cross-theory influences between psychology and neuroscience go back and forth, resulting in refinement in both disciplines. We interpret this case study as showing that: (1) Mutual co-evolution of psychological and neurobiological theories, exemplifying persisting top-down influences from psychology, is a more empirically adequate way to describe psychoneural theory relations than a view on co-evolution, favoured by reductionists, which regards the cross-theory contributions from psychology as merely heuristically useful with no enduring influence on neurobiological theorizing; (2) In research on vision, discovering (or hypothesizing) the neural basis of functions vindicates psychological approaches, it does not eliminate them; (3) Current work on vision shows that many perceptual phenomena must be understood in terms of dynamical interactions between an observer and his/her environment. Therefore, we argue that internalist characterizations of the visual system must be supplemented with externalist accounts that address these reciprocal observer-environment interactions involved in vision. Such processes seem quite different from (internal) cellular and molecular ones, and as such seem to lie outside the scope of neuroscientific inquiry. We conclude that psychoneural reduction or elimination is implausible as a meta-theoretical prediction of theory choice in empirical work. Instead, this case study of vision shows that both psychology and neuroscience contribute to, and complement one another in the study of visual perception. (shrink)

The earlier volume in this series dealt with two religions of Indian origin, namely, Buddhism and Jainism. The Indian religious scene, however, is characterized by not only religions which originated in India but also by religions which entered India from outside India and made their home here. Thus religious life in India has been enlivened throughout its history by the presence of religions of foreign origin on its soil almost from the very time they came into existence. This volume covers (...) three such religions—Zoraoastrianism, Judaism, and Islam. In the case of Zoraostianism, even its very beginnings are intertwined with India, as Zoroastrianism reformed a preexisting religion which had strong links to the Vedic heritage of India. This relationship took on a new dimension when a Zoroastrian community, fearing persecution in Persia after its Arab conquest, sought shelter in western India and ultimately went on to produce India’s pioneering nationalist in the figure of Dadabhai Naoroji ( 1825-1917), also known as the Grand Old Man of India. Jews found refuge in south India after the destruction of the Temple by the Romans in 70 C.E. and have remained a part of the Indian religious scene since then, some even returning to Israel after it was founded in 1948. Islam arrived in Kerala as soon as it was founded and one of the earliest mosques in the history of Islam is found in India. Islam differs from the previously mentioned religions inasmuch as it went on to gain political hegemony over parts of the country for considerable periods of time, which meant that its impact on the religious life of the subcontinent has been greater compared to the other religions. It has also meant that Islam has existed in a religiously plural environment in India for a longer period than elsewhere in the world so that not only has Islam left a mark on India, India has also left its mark on it. Indeed all the three religions covered in this volume share this dual feature, that they have profoundly influenced Indian religious life and have also in turn been profoundly influenced by their presence in India. BackgroundIn the Canadian Alliance for Healthy Hearts and Minds (CAHHM) cohort, participants underwent magnetic resonance imaging (MRI) of the brain, heart, and abdomen, that generated incidental findings (IFs). The approach to managing these unexpected results remain a complex issue. Our objectives were to describe the CAHHM policy for the management of IFs, to understand the impact of disclosing IFs to healthy research participants, and to reflect on the ethical obligations of researchers in future MRI studies.MethodsBetween 2013 and 2019, 8252 participants (...) (mean age 58 ± 9 years, 54% women) were recruited with a follow-up questionnaire administered to 909 participants (40% response rate) at 1-year. The CAHHM policy followed a restricted approach, whereby routine feedback on IFs was not provided. Only IFs of severe structural abnormalities were reported.ResultsSevere structural abnormalities occurred in 8.3% (95% confidence interval 7.7–8.9%) of participants, with the highest proportions found in the brain (4.2%) and abdomen (3.1%). The majority of participants (97%) informed of an IF reported no change in quality of life, with 3% of participants reporting that the knowledge of an IF negatively impacted their quality of life. Furthermore, 50% reported increased stress in learning about an IF, and in 95%, the discovery of an IF did not adversely impact his/her life insurance policy. Most participants (90%) would enrol in the study again and perceived the MRI scan to be beneficial, regardless of whether they were informed of IFs. While the implications of a restricted approach to IF management was perceived to be mostly positive, a degree of diagnostic misconception was present amongst participants, indicating the importance of a more thorough consent process to support participant autonomy.ConclusionThe management of IFs from research MRI scans remain a challenging issue, as participants may experience stress and a reduced quality of life when IFs are disclosed. The restricted approach to IF management in CAHHM demonstrated a fair fulfillment of the overarching ethical principles of respect for autonomy, concern for wellbeing, and justice. In this co-authored article, one contributor presents a case story from an interview with a social worker in Slovenia, while five others offer commentaries on ethical aspects of the case. The story comes from a practitioner working with a pregnant young woman, arranging for adoption following birth. The social worker respected the woman’s request to keep her identity secret, hence not registering her in the institutional records. However, whilst the social worker was on holiday, the baby was born and anonymity (...) was not maintained. Commentaries 1 and 2 evaluate the story through its form: as a narrative with a tempo and plot; and as a performance that creates its narrator as an agent with an ethical identity. Commentary 3 uses a normative moral philosophical framework (virtue ethics), while the final two commentaries take a more grounded approach. Commentary 4 views the social worker as using discretion to act in a space void of rules (there is no provision for anonymous birth), whereas Commentary 5 foregrounds the Slovenian code of ethics as a source of ethical standards unremarked upon by the social worker. The article ends with reflections on the value of exploring multiple perspectives and engaging in dialogue in developing ethical understandings and actions. (shrink)

Climate anxiety refers to persistent, difficult-to-control apprehensiveness and worry about climate change. Research to better understand the prevalence, indicators, causes, and consequences of climate anxiety is needed, to which emotion researchers can make substantial contributions. First, emotion theory can inform an integrative and functional theory of climate anxiety, mapping interactions between its cognitive, emotional, behavioural, and physiological indicators. Second, appraisal theories can help to understand the reasons why people experience climate anxiety. Third, emotion researchers can contribute to theorizing when climate (...) anxiety motivates climate action, accounting for non-linearity, interactions with other emotions and cognitions, and temporal dynamics. Fourth, emotion researchers can contribute to developing strategies to cope with climate anxiety, for example, by building on emotion regulation theory. (shrink)

PurposeTo explore the mindfulness practice, its long-term effects, facilitators and barriers, in clinical clerkship students 2 years after participation in an 8-week mindfulness-based stress reduction training.MethodA qualitative study was performed by semi-structured in-depth interviews with 16 clinical clerkship students selected by purposive sampling. Students had participated in a MBSR training 2 years before and were asked about their current mindfulness practice, and the long-term effects of the MBSR training. Thematic analysis was conducted using the constant comparison method. Data saturation was (...) reached after 16 interviews.ResultsMost interviewees were still engaged in regular, predominantly informal, mindfulness practice, although some discontinued mindfulness practice and reported an “unchanged lifestyle.” Three main themes came forward; “focused attention and open awareness” during daily activities as core elements of long-term mindfulness practice; “changes in behavior and coping” that resulted from taking a pause, reflecting, recognizing automatic behavioral patterns and making space for a conscious response; “integration in personal and professional life” by enhanced enjoyment of daily activities, improved work-life-balance and making different career choices. Barriers and facilitators in starting and maintaining mindfulness practice were understanding and intention as “pre-conditions”; practical, personal, and professional factors of students in maintaining practice.ConclusionTwo years after participation in a MBSR training, many interviewees were still engaged in mindfulness practice contributing to both personal and professional changes. In light of the high clerkship demands, MBSR training could be a valuable addition to medical curricula, supporting medical students in developing necessary competencies to become well-balanced professionals. In this series of papers I attempt to provide an answer to the question how the Babylonian scholars arrived at their mathematical theory of planetary motion. Papers I and II were devoted to system A theory of the outer planets and of the planet Venus. In this third and last paper I will study system A theory of the planet Mercury. Our knowledge of the Babylonian theory of Mercury is at present based on twelveEphemeridesand sevenProcedure Texts. Three computational systems of (...) Mercury are known, all of system A. System A1is represented by nineEphemeridescovering the years 190 BC to 100 BC and system A2by twoEphemeridescovering the years 310 to 290 BC. System A3is known from aProcedure Textand from Text M, anEphemerisof the last evening visibility of Mercury for the years 424 to 403 BC. From an analysis of the Babylonian observations of Mercury preserved in theAstronomical DiariesandPlanetary Textswe find: (1) that dates on which Mercury reaches its stationary points are not recorded, (2) that Normal Star observations on or near dates of first and last appearance of Mercury are rare (about once every twenty observations), and (3) that about one out of every seven pairs of first and last appearances is recorded as “omitted” when Mercury remains invisible due to a combination of the low inclination of its orbit to the horizon and the attenuation by atmospheric extinction. To be able to study the way in which the Babylonian scholars constructed their system A models of Mercury from the available observational material I have created a database of synthetic observations by computing the dates and zodiacal longitudes of all first and last appearances and of all stationary points of Mercury in Babylon between 450 and 50 BC. Of the data required for the construction of an ephemeris synodic time intervals Δt can be directly derived from observed dates but zodiacal longitudes and synodic arcs Δλ must be determined in some other way. Because for Mercury positions with respect to Normal Stars can only rarely be determined at its first or last appearance I propose that the Babylonian scholars used the relation Δλ = Δt −3;39,40, which follows from the period relations, to compute synodic arcs of Mercury from the observed synodic time intervals. An additional difficulty in the construction of System A step functions is that most amplitudes are larger than the associated zone lengths so that in the computation of the longitudes of the synodic phases of Mercury quite often two zone boundaries are crossed. This complication makes it difficult to understand how the Babylonian scholars managed to construct System A models for Mercury that fitted the observations so well because it requires an excessive amount of computational effort to find the best possible step function in a complicated trial and error fitting process with four or five free parameters. To circumvent this difficulty I propose that the Babylonian scholars used an alternative more direct method to fit System A-type models to the observational data of Mercury. This alternative method is based on the fact that after three synodic intervals Mercury returns to a position in the sky which is on average only 17.4° less in longitude. Using reduced amplitudes of about 14°–25° but keeping the same zone boundaries, the computation of what I will call 3-synarc system A models of Mercury is significantly simplified. A full ephemeris of a synodic phase of Mercury can then be composed by combining three columns of longitudes computed with 3-synarc step functions, each column starting with a longitude of Mercury one synodic event apart. Confirmation that this method was indeed used by the Babylonian astronomers comes from Text M (BM 36551+), a very early ephemeris of the last appearances in the evening of Mercury from 424 to 403 BC, computed in three columns according to System A3. Based on an analysis of Text M I suggest that around 400 BC the initial approach in system A modelling of Mercury may have been directed towards choosing “nice” sexagesimal numbers for the amplitudes of the system A step functions while in the later final models, dating from around 300 BC onwards, more emphasis was put on selecting numerical values for the amplitudes such that they were related by simple ratios. The fact that different ephemeris periods were used for each of the four synodic phases of Mercury in the later models may be related to the selection of a best fitting set of System A step function amplitudes for each synodic phase. A phrase like 'semantic theory of truth' evokes associations with rather recent developments in logic, especially the work of Alfred Tarski. Nevertheless, it is generally agreed that Hobbes made important observations of a semantical nature. Moreover, in an interesting article still (...) frequently referred to, R. M. Martin has argued that Hobbes "has in mind a clear-cut semantical concept of truth."' Examining Hobbes's semantical ideas somewhat more closely than Martin did, T6rnebohm has given a more precise characterization of Hobbes's view on truth and falsity; he speaks of Hobbes's comprehension theory of truth. ~One of the main purposes of this article is to investigate whether such a theory is correctly attributed to Hobbes. Moreover, in the case of linguistic expressions which are (deciarative ) sentences this semantic value should be its truth-value. 4 As we shall see, Hobbes's theory of propositions is basically a theory of subject-predicate propositions. It is therefore reasonable to qualify his theory of truth as a semantic one only if it satisfies Frege's principle to a certain extent. Ideally, this means that the truth-value of subject-predicate propositions should be conceived as a function of the semantic interpretations of its subject and predicate. In the next three sections I will concentrate principally on the logicosyntactical aspects of language. I will then discuss successively the general structure of propositions, the various sorts of names, and the syntax of Hobbes's theory of propositions. Section 5 is devoted to Hobbes's theory of truth as interpreted by Martin and T0rnebohm. The outcome of this examination makes it necessary to break away from Martin's restricted conception of semantics. Martin, like Tarski, takes semantics as "semantics in the narrower sense," i.e., as the discipline Quine refers to as the theory of reference. 5 However, sometimes this term is used in a broader sense; the field of semantics is taken then to include the theory of meaning too. In the Computatio sive Logica, the first part of De Corpore, containing Hobbes's most mature work on the philosophy of language, this opposition is displayed by way of a distinction between, on the one side, nominare, denotare and appellare and, on the other side, significare. Suerbaum concludes that this character's narration is not essentially different from that of the primary narrator of the Odyssey . Even though Odysseus is a first-person narrator and hence is subject to certain restrictions, these are almost completely counterbalanced by his ex eventu knowledge. For example, he can even report a conversation which took place on Olympus , because it was afterwards reported to him by Calypso, who heard (...) it from Hermes .
